The name of the lodge can not exceed 25 spaces. This includes any spaces or dashes between words in the name of your lodge.
The state will always be Kansas or KS.
The lodge number will be the number that the State Secretary assigns to your lodge.
The date of formation will be the charter date.
The meeting day will be something like the third Wednesday of each month.
The meeting place will normally be the address of one of the members unless you have a lodge hall.
The Total Members is for the total number of members in your lodge.
The State percapita tax is currently $12.50 for half a year per member.
The Grand (National)Lodge percapita tax is currently $3.25 for half a year per member.
The mailing address of the local secretary should not be a post office box. If it is, then a UPS delivery address should be included also.
