The Gains report has room for six new members on each page. Either printout a blank form and then fill it out, or load the form on your computer, fill it out, save it, and then print it out and mail it in with the check.

The full name should be listed in the Name space.

The blocks for Federal, State, City, & County refer to the type of department where the member belongs.

The address block refers to the street address of the member.

The member number should be left blank for new members. In the case of a member that transfers in from another lodge, provide the national number in that box.

The reason for new members is N. The reason for transfer members is TI.

Suspending members for non payment of percapita on a local basis is recommended rather than doing it on a national basis. It is quite frankly too hard to track. Wait until your bylaws provide that a member can be dropped, and then use the loss form to drop the member. (This is why the REIN catagory is listed on the form).
